Session 2: Case Management Fundamentals: Intake, Assessment, and Housing Plans

This session will provide an overview of the process of supporting a client from start to finish. The overview will start with client identification and go through intake, assessment, crisis response, service planning, documentation, evaluation, and housing plan development.
Learning Objectives
1. Participants understand strategies for engagement, intake, assessment, and housing planning using trauma-responsive strategies.
2. Participants are able to differentiate roles and responsibilities within the homeless response system for initial contact, triage, intake, housing assessment, housing planning, warm hand-offs among crisis response providers, navigators, and housing stabilization staff.
The training will be held virtually over Zoom.
